Reach South Sheffield is recruiting a dedicated and highly motivated Health Practitioner to work with local people to improve their health and wellbeing and link with groups and activities in the area.
The role will focus on engaging with individuals in communities and offering them practical support to change their behaviour to achieve healthier lives through the provision of social prescriptions.
This role will be based in Lowedges and will cover Lowedges, Batemoor, Jordanthorpe, Beauchief and Greenhill. The post holder will work with community members, GPs, voluntary sector organisations and Sheffield City Council.
The post will contribute to tackling inequalities in health through promoting and supporting people to develop healthier behaviours and lifestyles in the context of their own local communities. It focuses on engaging with individuals in communities and offering them practical support to change their behaviour to achieve healthier lives through the provision of social prescriptions.
The ideal person will be a genuine self-starter with bags of energy, enthusiasm and creativity and with a background in voluntary or community work.
