Senior Wellbeing Coach

Full-time: 37 hours per week

Salary: £24,098 – £26,357 per annum (dependent on experience and qualification)

Job purpose

The Senior Wellbeing Coach will jointly oversee the day to day running of the Wellbeing Coach team. Taking a lead on the operational delivery, management and development of Wellbeing Coach service, namely in partnership with SAPA5 Primary Care Network (PCN).

They will also be supporting individuals with chronic pain, working on a one-to-one and/or group basis to:

  • Provide tailored coaching support to help individuals develop healthier behaviours and lifestyles
  • Deliver health and wellbeing sessions aimed at promoting awareness and self-management of chronic pain and associated conditions
  • Co-produce a personalised support plan that encourages the empowerment of an individual to take control of their own health and wellbeing
  • Work in partnership with clinical and non-clinical staff which form part of the PCN’s Personal Care team
  • Develop close working partnerships with health, social care, housing and voluntary, community or faith (VCF) organisations.

Family Welfare Coach

Full-time: 37 hours per week (30 hours upwards considered)

Salary: £21,085 – £23,345 per annum (dependent on experience and qualification)

Permanent (subject to funding)

Job purpose

The Welfare Coach forms a key part of SOAR’s Social Prescribing Service. Working intensively with families who have a range of complex needs, the role will:

  • Provide tailored emotional and practical support to help individuals experiencing financial and social hardship, achieve a ‘simpler life’
  • Co-ordinate, simplify and broker access to a range of health and social interventions based on the needs and wishes of the individual, including liaising with a range of agencies (schools, housing, MAST)
  • Empower the individual to express their views, wishes and concerns
  • Co-produce a personalised support plan which encourages the empowerment of an individual to take control of their own health and wellbeing
  • Develop close working partnerships with health, social care, housing and voluntary, community or faith (VCF) organisations.
